I've monitored Ping Plotter for several days now and have noticed on every day that the second address listed claims it has 100% packet loss, but no IP or DNS Name are listed.

What is this second address? How would I go about fixing the packet loss?

This is pretty normal. Hop 2 is probably your DSL modem, Cable modem, or similar. Hop 3 looks just fine, so it's just the way hop 2 is configured. We have some knowledge base articles that cover this topic here:

The picture you post here looks pretty perfect. 20ms of latency, no packet loss at the final destination.
